New Dental Patient Information

We Make Preparing for Appointments Easy

Dr. Vatavuk is honored that you’re considering our dental office for all your oral healthcare needs. As you prepare for your first appointment with us, we recommend that you take a moment to set expectations beforehand and consider completing a few simple steps to make your visit as smooth as possible. Below, you can learn how we introduce new patients to the dental office, fill out essential patient documents, and learn about your financing options.

Your First Visit

After you arrive at our dental office, you’ll be given a series of new patient forms if you did not fill them out already prior to your arrival. You’ll be brought to an operatory to meet with Dr. Vatavuk, who will confirm any concerns you have currently about your oral health and perform a detailed exam with these in mind. He always dedicates enough time to get to know you so you can feel comfortable about your future care. With time permitting, you’ll have a cleaning completed as well. If we recommend a treatment plan, you’ll have it explained to you in full with our front desk. From there, we’ll schedule your next appointment.

New Patient Forms

Our new patient forms provide our dental office an opportunity to get to know your needs better. While they ask questions about your current oral health and contact information, they also provide you a chance to tell us about any other accommodations you’d like us to make or medications you are currently taking. To expedite your check-in process, feel free to download, print, and complete your new patient forms, then bring them to our dental office for your first visit.

Dental Insurance

With a dental insurance plan, paying for your preventive dental services can become easier and more affordable. However, since insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. We currently accept assignment of benefits and will bill out your insurance, but you will need to pay any remaining difference. If you have concerns about your plan, we ask that you call or contact us with specific questions or have them in mind when you visit.

Financing Available

If you do not currently have a dental insurance plan and are looking for an alternative financing option, we suggest taking a look at third-party solutions such as CareCredit. This interest-free (in many cases) financing option allows you to pay for treatments over several months, rather than in one lump sum. This makes even the most expensive dental treatments more manageable on a fixed budget. To learn more about the approval process or complete an application, please give us a call!
